Important Information For Current and Former Patients

K Health will discontinue the Platform and access to telehealth providers as of December 31, 2025. Here is what you need to know

When will my access to clinicians on the Platform end?

You will be able to schedule visits for ongoing and new issues between now and December 22, 2025. After December 22, 2025, clinicians will only be available for outstanding issues arising from ongoing care.

Where do I find Care After December 22, 2025?

We suggest that you plan ahead to find other care so you will not experience a gap in your care once clinical services end on the Platform. We have provided some resources you can use to find other providers for your care below. Resources to Find Care

How do I Make Sure I don’t run out of my prescription medications?

If you are receiving ongoing medications prescribed by a K clinician, you will want to find replacement care so you don’t experience a lapse in your care. If you need additional medicine before you find your replacement care, you need to schedule an appointment before December 22, 2025 to discuss getting refills. Any prescriptions must be clinically appropriate and consistent with your health insurer’s refill policies, if applicable. No refills will be available after 12/31/2025.

Can I transfer my medical records to my new provider?

Yes, you can. To access your medical records, please go to the K Health app, click on “Profile” at the bottom right, and click on “Download medical records”. Once received, please review your medical record. You may be missing older lab reports or reports of referrals or images. If you are missing any information, send an email to [email protected] requesting the additional information and specifying a secure email address where it can be sent.

Will you retain my medical records after December 31, 2025?

Yes. We will arrange for your records to be available as needed for the period required by applicable law. Following that date, the records will be securely destroyed.

Can I still log into the app and see records of my previous visits after December 31, 2025?

After December 31, 2025, the app will no longer be available. To request a copy of your medical record after December 31, 2025, go to the medical records address here and follow the process to access your records.

Please download and retain this for your future reference. We thank you for your trust in K Health and its clinicians and wish you the best of health.
