K Health discontinued the K Health Platform as of December 31, 2025. We continue to offer care through our health system partners. Here is what you need to know:

How can I access or transfer my medical records?
You can directly download and access your medical records by logging into this secure site using the same credentials you used to log into the app. Once downloaded, you can send to any third-party or provider of your choice.

Yes. We will arrange for your records to be retained and available as needed for the period required by applicable law.

- The primary way to find care is by using the Surest app or by logging in to [www.Surest.com](https://w
- The app and website are designed for "shopping" for care by condition, symptom, or doctor.
- You can search for a general symptom like "back pain" or a specific provider name. The results will show yders with their upfront costs (copays).

- If you cannot find what you are looking for on the app or website, or if you need to confirm a provider's Surest Member Services.
- The number is 866-683-6440. You can also find this number on the back of your Surest member ID card.
