Everyone with asthma needs his or her own Asthma Action Plan! An Asthma Action Plan is a written plan to help control your asthma.

What Is An Asthma Action Plan?

Attached is a sample Asthma Action Plan that includes both long-term control (preventive) medicines to take every day, and quick-relief (rescue) medicines for when symptoms are getting worse.

It also provides guidance on when to call your healthcare provider or go to the emergency room.
